Health insurances accepted: Medicare (and secondaries such as Tricare, AARP, Plan 65 etc), Blue Cross/Blue Shield, United Healthcare and Aetna
If we do not accept your insurance, payment is expected at the time of the visit. Please bring your insurance card with you to each and every appointment.
Copayments are required at the time of service.
Cosmetic treatments are generally not covered by health insurance. Payment is expected at the time of service.
Cancellation policy: We reserve the right to charge a fee for missed appointments, or for appointments which are not cancelled 24-hours in advance.
